Scroll to the bottom for the registration.REGISTRATION FOR DAILY ATTENDANCE TRACKING ONLYUse this registration to report in your child's attendance to THE CLUB After-school Program. You MUST "CHECK OUT" as if you are paying for the items in your cart- COST IS $0. You know when it's fully submitted when the last screen you are on says "THANK YOU".The attendance report is due no later than the last day of the month for the next month.
